However, he likes to see me lose a shoe, heel stuck etc. Should I worry about this? Has anyone come across this before?

I dunno if it's a problem. Sounds like he's got Cinderella fantasies or wants to play the hero or Prince Charming. Doesn't sound horrible or sinister. Talk to him, if you can. Maybe he'll reveal his motives for liking this sort of thing.
